Can Apple Watch Measure Cortisol?

No. Apple Watch cannot measure cortisol levels. It records heart data, including heart rate variability (HRV), that apps can use to estimate physiological stress. A stress score does not tell you how much cortisol is in your body.

If you searched for a way to track cortisol on Apple Watch, the distinction matters: the watch can help you follow patterns in your heart data, but checking a hormone level requires a different kind of measurement.

What Apple Watch measures

Apple lists heart sensors, temperature sensing and other health features in its Watch specifications. There is no cortisol sensor or cortisol measurement feature. Installing an app does not add that measurement to the watch.

HRV describes the variation in time between heartbeats. One HRV metric recorded in Apple Health is SDNN, expressed in milliseconds. Apple's HealthKit documentation explains how it is calculated.

Sleep and overnight readings can add context. Depending on your watch and software, available data include heart rate, respiratory rate and wrist temperature. Apple explains the requirements and availability in its Vitals guide. None of these values is a cortisol concentration.

Does low HRV mean high cortisol?

You cannot work out your cortisol level from an HRV reading. HRV reflects influences on your heart rhythm, including activity of the autonomic nervous system. Cortisol is a hormone produced by the adrenal glands. Both are studied in relation to stress, but they are different measurements.

A review of research on HRV and psychological stress found changes in HRV during stress and discussed its potential as a stress indicator. That does not establish a conversion from milliseconds of HRV to a cortisol level, or make an individual reading a diagnosis.

How is cortisol actually tested?

Cortisol tests use a sample of blood, saliva or urine. Levels vary during the day, so the type of test and the time of collection affect interpretation. A healthcare professional can decide which test is appropriate and explain the result. MedlinePlus describes these tests and their uses.

A watch reading cannot confirm high or low cortisol. If you are concerned about a hormone condition or persistent symptoms, discuss them with a healthcare professional rather than using a stress score to decide whether you need testing.

Can an Apple Watch app calculate my cortisol level?
